<p>A veneer is a tough but ultra thin wafer of porcelain.  When placing a veneer on a tooth, the tooth usually needs to be ground down. This prepares the tooth to adhere to the veneer with the help of a bonding agent</p>

<p>Eve has multiple problems with the function of her teeth because she is a “teeth grinder”.  Some people grind their teeth so much that they wear them down at a faster pace the normal.  Everyone looses tooth structure as they age but Eve has done a lot of damage to her teeth by grinding her teeth at night.</p>
<h2>TMJ Pain</h2>
<p>When a patient grinds their teeth, they put a lot more pressure on their jaw joint and facial muscles.  The pain can radiate to the head causing headaches.  Many patients do not think their headaches are cause by teeth grinding, as they say it is a “sinus headache” or “stress”.</p>

<h2>How Tetracycline Stains got started</h2>
<p>Tetracycline stains are unfortunately the most problematic and difficult to manage and remove. Many years ago tetracycline was prescribed by doctors as an effective antibiotic for treating many illnesses. It was even perceived to be a more effective treatment than penicillin. The devastating long term cosmetic effects on teeth enamel were not known at the time.</p>
<p>Many of today&#8217;s adult population who received tetracycline treatment are now faced with teeth stains that are seemingly unmanageable and frustratingly untreatable.</p>
<h2>Tetracycline Research</h2>
<p>Various dentistry professionals and researchers discovered that tetracycline staining actually becomes darker as the tooth is penetrated below the surface.</p>
<p>Endodontic specialists at USC discovered an effective method of tooth whitening that involved bleaching each tooth &#8211; starting from the inner part of the tooth and bleaching &#8220;outwards.&#8221; A fairly invasive technique however that required a complete root canal on each affected tooth. For many people this would be unfeasible.</p>
<h2>Successful Tetracycline Treatment</h2>
<p>The young lady in the adjacent photo expressed a desire of treating her severely stained teeth. She too received tetracycline treatments as a youngster&#8230; for an extended time. All of her teeth had a &#8220;stain band&#8221; (see photo&#8217;s below for detail)</p>
<p>Her primary goal was to have the &#8220;whitest of white&#8221; teeth. Dr. Muslin determined that her teeth would respond to a &#8220;customized&#8221; application of teeth whitening procedures.</p>
<h3>Different Patients &#8211; Different Teeth Whitening</h3>
<p>No two patients will respond identically to a particular teeth whitening procedure. Dr. Muslin uses any combination of BriteSmile, Zoom, Crest Whitening Strips, Rembrandt and laser teeth whitening methods that will be effective for the individual patient.</p>
<h2>Tetracycline Bleaching &#8211; Cosmetic Dentistry</h2>
<p>During the patient&#8217;s consult with Dr. Muslin she learned that additional procedures could help her address some other cosmetic concerns. She had gaps between her upper teeth and also had teeth that were too narrow. Crowns that were placed by another dentist were too dark. Lastly, gum-line symmetry was not balanced.</p>
<p>It was decided by the patient that she would not only acquire and ultra bright smile but undergo some popular cosmetic procedures incorporating the use of porcelain materials to match the whiteness achieved on her natural teeth.</p>
<p>The first step, before selecting the porcelain materials, was to achieve the highest degree of whiteness for all of her teeth. Dr. Muslin used laser teeth whitening procedures to obtain the shade of whiteness desired by the patient. Her teeth responded well to the laser treatments.</p>
<p>Porcelain crowns were recommended by Dr. Muslin to widen the narrow appearance of her smile&#8230; thereby closing up the apparent gaps in her upper teeth. Dr. Muslin was able to help the patient understand that the net effect would be an even broader, natural looking and whiter smile which would certainly enhance her attractiveness.</p>
<h2>Precision Dental Ceramics</h2>
<p>Matching the color of the new porcelain materials to the patient&#8217;s new whiteness level of her lower teeth was no small task&#8230; requiring close coordination between the patient, Dr. Muslin and his ceramist.</p>
<p>This extra effort had a very dramatic impact on this reconstructive effort, as can be seen in this second photo. The patient overwhelmingly achieved her primary goal of having the &#8220;whitest of white&#8221; teeth.</p>
<p><strong>No unsightly stains</strong>. The gum reshaping and re-contouring produced a symmetrical gum line that was desired by the patient. Precision ceramics produced a smile line (the arc of the lower biting edge of the upper teeth) that established the final &#8220;harmony&#8221; of her smile (the pictures tell the story).</p>

<p>Reconstructive dentistry can applied, given the requisite talent,  experience and access to advanced dental technologies, toward a number of dental conditions. In the photo here the patient has a lower jaw that appears small and pushed back.</p>
<p>This is caused by an abnormal positioning of the chin due to a significant problem with <strong>occlusion</strong> (or bite &#8211; how the teeth meet when the jaw is closed).</p>
<p>As can be seen in the closeup of the teeth, the relative positioning of the teeth and jaw is far from normal. The close up pictures reveal a deep overbite.  That means the upper teeth completely cover his lower teeth when the patient bites together.</p>
<p>Many people have various degrees of overbites. Improperly managed or monitoring of the bite relationship can eventually, if untreated, affect the entire face!</p>
<p>In severe or prolonged cases <strong>Orthognathic Surgery</strong> may be performed. The procedure involves breaking the jaw structures at critical points that enables surgeons to reposition the jaw to create the bite that is most appropriate for the patient&#8217;s jaw structure.</p>
<h2>Occlusion Analysis</h2>
<p>Before meeting with Dr. Muslin this young man was turned away by orthodontists in the area. Using special x-rays and &#8220;bite &#8211; occlusion analysis&#8221; Dr. Muslin was able to develop a treatment plan that addressed a variety of problem areas.</p>
<p>After determining the jaw and chin structures were of normal size, Dr. Muslin created custom orthodontic appliances (braces) that were positioned on the inside, or tongue side of the patient&#8217;s teeth. These special braces were worn for one year. During the second year braces were worn on the outside of the teeth.</p>
<p>During treatment Dr. Muslin performed dental surgery to certain teeth to overcome potential difficulties with &#8220;eruption&#8221; of a canine tooth. The surgical intervention was overwhelmingly successful.</p>
<h2>Reconstructed Normal Occlusion</h2>
<p>As this profile photo plainly demonstrates, occlusion and orthodontic factors have a significant impact on appearance and can affect future growth patterns. The deep overbite has been successfully treated.</p>
<p>This young man now has a normally positioned lower jaw and chin. His bite is normal and enjoys having a beautiful, symmetrical smile and smile line.</p>
<p>Technology was used in the diagnosis and many years of experience in treating faces gave Dr. Muslin the ability to deliver the highest level of quality for this patient. <p>The &#8220;makeover experience&#8221; is now a common desire for patients who want great looks in almost an instant. There has also been a similar effect for dentists. Many practitioners are taking crash courses in veneer technologies so they can better serve their patients.</p>
<p>This frenzy however has caused an alarming number of treatment failures that essentially stem from the lack of diligence on the part of patients&#8230; and on the part of dentists.</p>
<h2>Veneer Bonding Problem Failures</h2>
<p>Stories of veneers popping off, cracking and causing discomfort (severe in some cases) are beginning to accumulate nationwide. As with most components used in restorative and reconstructive dentistry, veneer laminate failures are usually not related to specific product failures&#8230;. but rather a failure of application or technique.</p>
<p>Premium lamination procedures are technique specific and require <strong>substantial experience</strong> in the use of different types of veneer products for different types of lamination needs that can differ quite significantly on a tooth by tooth basis for all patients.</p>
<p>There is no single veneer product that can fulfill every veneer need. Popular veneer products have different structural and cosmetic qualities that must be used effectively and creatively by dentists so certain functional and cosmetic goals are obtained&#8230;. as desired by patients.</p>
<p>Dr. Muslin uses a variety of popular veneer products&#8230; recommending specific products for whatever cosmetic goals are defined by the patient.</p>
<h2>Invasive versus Non Invasive Veneer Tooth Preparation</h2>
<p>A common complaint that surfaces with veneer dentistry is the experience of having teeth that mysteriously become ultra sensitive to temperatures and pressure after veneer treatment.</p>

<p>Some veneer products and/or veneer results (as desired by patients) require invasive reshaping of the underlying tooth structure. Inadequate or over preparation of tooth structures can create poor adhesive properties that become the basis for the complaints mentioned above.</p>
<p>The dentistry sciences have created a variety of lamination products and technologies that can successfully treat almost any appropriate veneer need.</p>
<h2>Most Common Cause of Failure</h2>
<p>As mentioned above.. failures tend to be technique specific rather than related to the brand of veneer product. Most failures are caused by errors or contamination in the <strong>bonding process</strong>. These types of errors are difficult to control or eliminate if a dentist does not have access to certain technologies.</p>
<p>Our high tech operatories practically eliminate bonding problems related to treatment technique or contaminants. In special situations where adhesive properties are critical (based on location or shape of tooth structure), Dr. Muslin has a proprietary formula for creating a bonding formula that assures successful treatment regardless of the uniqueness of the tooth structure.</p>
<p>To further control and limit veneer failures, most of our porcelain laminates are made through our private ceramist. Complaints of having fat, unattractive or goofy looking Chiclet style veneers do not occur. Errors in cosmetic appearance (e.g., not white enough.. or too white), by virtue of having immediate access to our lab resource, are corrected immediately and without fail.</p>
<p>Dr. Muslin&#8217;s access to and experienced use of proprietary bonding technologies has enabled him to correct veneer and generalized bonding failures (bonding is also involved in crowns) reported by patients.</p>

The high tech porcelains have different optical properties that reflect light and had dark colored teeth in different manners.  The optical properties of porcelain veneers that are matched to the needs of the patient produce ideal results.  Cynthia had very dark teeth and so did Lilly.  The colors of the old teeth had to be hidden so the right type of porcelain for the porcelain veneers was necessary to hide the dark colors.</p>

<h2>Reshaping the Smile Line</h2>
<p>Dr. Muslin is able to create truly dazzling, permanently white smiles by using custom thicknesses and shapes of feldspathic porcelain to create a cosmetic result that feels and looks natural. As the adjacent photo shows&#8230;.this adult patient had great looking teeth that were quite healthy.</p>
<p>She did not like however the overall shape of her teeth. In some instances she also felt that some of her teeth lacked a certain bulk, or thickness.</p>
<p>This after photo reveals how dramatic veneer bonding can be. Most of the upper arch teeth now have the appearance of being fuller (more bulk) and do not taper inward. Gum reshaping and contouring was also performed to produce a very natural appearing harmony across all of the gingival tissues.</p>

Implants can be used for single teeth or multiple teeth. Specialized implant devices are also used for providing anchoring systems for bridgework, when a patient&#8217;s existing tooth structures are not suitable as anchors.</p>
<p>Specialized hybrid implants have been recently developed for stabilizing dentures where the use of a  normal implant is not feasible. These have become known as Mini Dental Implants.</p>
<h2>Jaw Bone Density &#8211; Critical Implant Issue</h2>
<p>Some patients seeking implant surgery have jaw bone densities that can complicate the placement and use of certain types of implant devices.</p>
<p>Bone tissue can deteriorate for a number of reasons, the most popular being due to a missing tooth (older, existing extraction) or the lack of an opposing tooth structure. Opposing teeth serve to stimulate the immediate bone tissue that tooth structures are rooted in.</p>
<p>Other issues including congenital malformations (missing teeth), a variety of diseases, oral infections, unusual occlusal relationships and physical trauma can form the basis of compromised or weakened jaw bone structures, both mandibular and maxillary (lower and upper jaws, respectively).</p>
<h2>Bioengineering &#8211; CAD CAM Technologies in Reconstructive Dentistry</h2>
<p>Materialize (SimPlant) and Nobel Biocare (NobelGuide) have developed complex technologies for overcoming a large number of bone density issues. CT Scan based images are capable of determining the integrity status of jawbone material with scientific accuracy. These images, when coupled with proprietary software developed by these two different firms, are used to develop specialized technologies for locating the best and most viable surgery sites for each type and placement of an implant.</p>
<h2>SimPlant Guided Surgery Implant Solutions</h2>
<p>In our office, Dr. Muslin uses Simplant technologies for achieving treatment success for a variety of implant problems. Bone density or inadequate bone structure issues no longer present problems as they had in the past, for some patients.</p>
<p>Patients who have been told they were not candidates for implants discover, in some cases, that implants are indeed possible&#8230; with specialized accurate placement. In other treatment plans, specialized implant systems can be devised that incorporate the best advantages of existing bone structures that are used to create a variety of methods for replacing teeth.</p>
<h2>Site Preparation &#8211; Implant Accuracy</h2>
<p>Viable implant sites, with the aid of computer generated guides, offer the precision for accurate implant site preparation that cannot be matched by &#8220;free hand drilling&#8221; (no guide or template is used) that some doctors perform. Correct bore, depth and selection of the &#8220;correct&#8221; implant is thereby assured.</p>
<h2>Implant Guide Types</h2>
<p>Our patients have three (3) options for guided implant technology for single and multiple tooth structures. The first, is a guide that slips directly over mucosa tissue.</p>
<p>A mucosa supported SurgiGuide is a custom manufactured drill guide made for a unique and stable fit on the soft tissue of the patients jaw. A popular technology based on its qualities of requiring <strong>minimally invasive surgery</strong></p>
<p>This type of SurgiGuide can be used for patients with edentulous jaws (toothless, perhaps desiring a specialized denture).</p>
<p>Another type of SurgiGuide is designed for placement directly on the bone structure. During surgery, a ridge incision is made and mucoperiosteal flaps are raised to free the bone surface. This type of guide is suited for edentulous and partially edentulous patients.</p>
<p>The third type of SurgiGuide is designed for more specialized implant applications that incorporate more unique implant systems such as the Zygoma implant.
